What it is
Assessment of how an adult manages daily life, and what would change it. The same five domains a funder reads for: daily living, mobility, communication, cognition and social participation.

- Function described across good days and bad, not just the day we visited
- Equipment and home modification recommendations scoped so they can be approved and actually happen
- Reports written for the decision they have to survive
Who it’s for
Adults on an NDIS plan, older adults through My Aged Care, people returning to work after injury or illness, and private clients.
What gets assessed
Most adult referrals come down to one question: can this person keep doing the things that matter to them, and what would have to change? The assessment is built around that rather than around a diagnosis.
- Daily living: showering, dressing, meals, medication, housework, shopping.
- Mobility and transfers: around the home, in and out of bed, chairs and cars, and out in the community.
- Cognition: planning, memory, initiation, safety awareness.
- Work and study, where those are part of the picture.
- The environment itself: doorways, thresholds, bathrooms, driveways. Equipment that suits the person but not the house comes straight back.
Where it leads
An adult assessment usually produces one or more of a functional capacity assessment, an assistive technology prescription, a home modification scope, or a return to work plan. For people going through the aged care system it is a My Aged Care assessment instead.
Turning 18 on an NDIS plan? Plans change shape at that point, and so does what the evidence has to show. If a young person is approaching that, see adolescent assessments: the handover is easier when it is planned rather than discovered.

Common questions
Who can request an adult assessment?
Anyone. Participants, family members, support coordinators, plan managers or GPs. No formal referral document is needed to start.
Do you come to the person, or do they come to you?
We travel. Assessment happens at home, at work or in the community, because that is where the difficulty shows up.
Which funding do you work with?
NDIS participants who are self-managed or plan-managed, older adults through My Aged Care, and private clients.
